How to obtain the service:
1. Prepare the documents required to fill out the application form, as well as documents confirming belonging to a social (special social) category.
2. It is necessary to fill out: the Application Form, indicating information about yourself, information about the person undergoing the examination, the purpose of the examination, as well as the personal data of the person being examined.
3. Upon completion of data entry, the system will process and send your request to government agencies for the stages of registration, verification of the application and adoption of a final decision.
4. If the service is successfully performed at all stages, the system will give you a positive response, and government agencies will inform you of the need to come to the territorial body of the department to receive the documents. If a refusal occurs at any stage of the service provision, the system will give a negative response, and government agencies will inform you of the reasons for the refusal.Ways to submit an application:

Result of providing the service:
Issuance of a certificate confirming the fact of establishment of disability, and an individual rehabilitation program for a disabled person (disabled child), as well as sending an extract from the examination report of a citizen recognized as disabled to the body providing their pension provision, sending the individual rehabilitation program for a disabled person to the territorial body of the Social Insurance Fund of the Russian Federation or to the executive authority of the constituent entity of the Russian Federation authorized to exercise, under an agreement concluded by the Ministry and the supreme executive authority of the constituent entity of the Russian Federation, the powers to provide social protection measures for disabled persons by providing technical means of rehabilitation, at the place of residence of the disabled person (disabled child); in case of refusal to establish disability - issuance of a certificate on the results of the medical and social examination (at the request of the recipient of the state service).
